Immunization Requirement Instructions for New Students University of Colorado Boulder www.colorado.edu/health/newstudents 1. Log in to the MyCUHealth Patient Portal (https://mycuhealth.colorado.edu)
5. In the drop down menu select “Immunization Record” then select your file and click Upload.
2. Click on the Immunizations tab and select Enter Dates.
Once you’ve successfully uploaded your file, you will see the file listed under Documents already on file. You can view this document at any time.
3. Scroll down to the Required Immunizations section and enter the dates of your first and second dose of the MMR vaccine as they appear on your immunization record, then click Submit.
6. Click on the Forms tab to complete the Meningococcal & TB Questionnaire.
4. Next, click on the Upload tab at the top to upload a copy of your immunization record.
If you are under 18 this form must be completed by a parent/guardian. Download and print from the Downloadable Forms section. Options for uploading your record: You can upload an electronic copy from your computer or take a photo of your record and upload it from your phone.
